PatientswithanyofthefollowingfeaturesmustbeevaluatedclinicallyforotherdiagnoseseventhoughIBSmaybepresent: Signsorsymptomsofgastrointestinalbleeding. Unexplainedirondeficiencyanemia. Unintentionalweightloss. Palpableabdominalmassorlymphadenopathyonexam. Familyhistoryofcoloncancerandhavenothadage-appropriatecoloncancerscreening. Onsetofsymptomsage≥50yearsandhavenothadage-appropriatecoloncancerscreening. Suddenoracuteonsetofnewchangeinbowelhabit. WhentoUsePearls/PitfallsWhyUsePatientspresentingwithsymptomssuggestiveofIBSforatleastthepast6months.ThediagnosisofIBSshouldbemadebyclinicalhistory,physicalexamination,andminimallaboratorytests.Colonoscopyorothertestsshouldbedoneifclinicallyindicated.PerformingabatteryoftestsinallpatientssuspectedofhavingIBSisnotwarranted. Thepresenceofchronic,recurrentabdominalpainatleast1dayperweekisarequirementtomakeadiagnosisofIBS.ThiscriteriawasdevelopedmainlytomakeamoreprecisediagnosisofIBSforclinicaltrials. Patientswhodon’tmeetfrequencycriteriaofabdominalpainatleast1dayperweekmaystillbetreatedasIBSinclinicalpractice. Ifpatientsdonothaveabdominalpainbuthavealteredbowelhabitswithoutasecondarycause,anotherdiagnosisshouldbeconsidered(e.g. functionalconstipation,functionaldiarrhea>,etc.). TheIBSsubtypeisdeterminedbasedonthepresenceofabnormalstoolsusingtheBristolStoolFormScale.Patientsshouldnotbeusingmedicationsthatcouldaffectstoolform. Forwomen,theabdominalpainshouldnotexclusivelyoccurduringtheirmenstrualperiod. UsedtoestablishadiagnosisofIBSinpatientswhopresentwithabdominalpainandalteredbowelhabits. Makingadiagnosiswillhelpguidetheneedforfurtherwork-upofsymptomsandmanagement. CanbeusedtodiscussthediagnosisofIBSwithpatientsandinstitutetreatment.
